Abstract

Hensikten med denne undersøkelsen var å belyse et lite studert område, nemlig: Stemmer enkeltvedtak overens med hjelpen hjemmeboende personer med demens får, og om tap av ferdigheter påvirker vedtakene og hjelpen som gis. Materiale: I alt 460 hjemmeboende personer med demens fra 24 kommuner i alle landets fem helseregioner ble inkludert i undersøkelsen. Alle fikk hjelp fra hjemmebaserte tjenester og hadde en demenssykdom diagnostisert av lege og/eller hjemmesykepleien antok at det forelå demens på grunnlag av en Klinisk Demensvurdering I alt 489 pasienter ble rekruttert, 249 med demensdiagnose og 240 uten. Blant de 240 med antatt demens hadde 211 en KDV skåre 1 eller høyere. Disse ble inkludert, mens de 29 med lavere skåre ble ekskludert. Metode: Pasienten ble vurdert med Klinisk Demensvurdering og et skjema som måler funksjoner i dagliglivet, Rapid Disability Rating Scale-2. Enkeltvedtak for pasientene ble samlet inn og kategorisert ved hjelp av kvalitativ innholdsanalyse slik at kategoriene stemte overens med hjelpetiltakene som ble tilbudt. Hjelp til pasientene ble registret i en sju dagers periode. Resultater: Vi fant liten sammenheng mellom vedtakene og utført hjelp, og liten sammenheng mellom behov for hjelp og utført hjelp. Åttiseks prosent av vedtakene ble fulgt opp av hjemmetjenesten, men svært mange får hjelp uten vedtak. Undersøkelsen viser at mange som har behov for hjelp, ikke får hjelp. Undersøkelsen viser at det ikke er lovverket som svikter, men håndhevelsen av gjeldende lover og forskrifter. The purpose of this study was to look more closely at a previously neglected area namely: Do administrative decisions for provisions of care correspond to the actual care given to persons with dementia living at home, and does the loss of skills effect the decision and care provided?The method used was collection of data, which was than categorised by a qualitative analysis of content. Although individual administrative decisions are governed by law, and most local authorities have passed guidelines on how these decisions should be formulated, and how the care should be provided, our study showed a large variation in the quality of provisions and professional practice. Four hundred and sixty persons with dementia, from all health regions, were included in the study. Inclusion criteria were patients with known dementia, diagnosed by a doctor and/or assumed to be demented by the community nurse. In total, 489 patients were recruited, 249 with a confirmed diagnosis of dementia, and 240 without. Of these 240 patients, with assumed dementia, 211 had a KDV score of one or greater. These were included, whilst the 29 with lower scores were excluded from the study. Data for the study was thus collected from 460 patients. The results show little relationship between the administrative decisions for care, the need for care, and actual care provided. Eighty-six percent of administrative decisions were followed up by the home services, but many receive help without formal decisions. The study also found many in need of help who did not receive it. Routines for arriving at and formalising administrative decisions were incomplete. The study shows a failure of implementation of existing laws, regulations and patient rights, rather than a failure of the laws and regulations themselves.

Abstract

Syfte. Avhandlingen syftar till att bidra med ökad kunskap om faktorer som kan antas påverka patientsäkerheten dels på system-, dels på individnivå. Metod.Det nationella rapporteringssystemet Lex Maria analyserades före och efter Socialstyrelsens regionalisering av tillsynsorganisationen 1990/91. I en av de två studierna analyserades särskilt ärenden som överförts till HSAN för prövning av disciplinär påföljd. I tre empiriska studier analyserades kompletterande metoder till rapportering för att få information om risker i hälso- och sjukvården. Personal har intervjuats om deras uppfattning av potentiella risker som skulle kunna hota patienternas säkerhet. Vårdbiträdens och undersköterskors kunskaper om diabetes undersöktes i en enkätstudie. Eventuella samband mellan vårdbiträdens och undersköterskors kunskapsnivå och medicinskt ansvariga sjuksköterskors uppfattning om säkerhetsrelaterade frågor undersöktes i en uppföljande enkätstudie. I en kvalitativ studie undersöktes flygets metoder för säkerhetsrelaterat arbete vilket jämfördes med motsvarande arbete inom hälso- och sjukvården. Resultat.Lex Maria-rapporteringen ökade efter regionaliseringen av tillsyns-verksamheten och en minskad andel ärenden överfördes till HSAN för disciplinär prövning. De kompletterande metoder för att identifiera risker som undersökts visade sig vara värdefulla tillägg till traditionell rapportering. Intervjuerna inom barnsjukvården uppvisade en annan riskbild än den som fås genom rapporteringssystemet Lex Maria. Enkäten till vårdbiträden och undersköterskor visade ett tydligt samband mellan bristande kunskap om diabetes och en ökad risk att vidta en riskfylld åtgärd. Studien indikerade brister på systemnivå, vilket bekräftades av resultaten från enkätstudien till medicinskt ansvariga sjuksköterskor i motsvarande kommuner. Studien visade på behovet av ökad uppmärksamhet på hur sjukvårdens stödsystem påverkar yrkesutövarna i frontlinjen. Jämförelsen mellan flyget och hälso- och sjukvården visade att flyget hade ett mera proaktivt och bredare förhållningssätt till säkerhet än vad hälso- och sjukvården uppvisade. Slutsatser. Studien har visat att yrkesutövare som befinner sig i frontlinjen och möter den enskilde vårdtagaren är särskilt sårbara när hälso- och sjukvårdens stödsystem uppvisar brister. Förebyggande säkerhetsanalyser som komplement till rapportering visade sig ge värdefull information för det förebyggande säkerhetsarbetet. En sammanhållen strategi baserad på aktuell säkerhetsforskning kan bidra till att utveckla hälso- och sjukvårdens säkerhetsarbete. Abstract

In de zorgvisie die vandaag nog vooropgesteld wordt door de zorginstellingen, blijft de patiënt vaak aan de kant staan met zijn/haar zorgvraag. Er wordt in zijn/haar plaats beslist door de zorgverleners en door de instelling, de patiënten worden vaak verplicht te 'ondergaan'. Anderzijds is de maatschappij vandaag zo geëvolueerd dat patiënten nu bewuste gebruikers zijn van zorg. Met andere woorden, ze zijn meer cliënt geworden binnen de zorgsector. Ze willen juist die zorg consumeren gericht naar hun zorgvraag, zorgbehoeften, voorkeuren en verwachtingen, niet meer of minder dan ze nodig achten. Patiënten wensen op een geïnformeerde basis mee beslissingen te nemen. Het boek geeft een compleet overzicht van de actuele visie wat betreft de zorgverlening in al haar facetten, durft hierop een kritische blik werpen en een aantal terechte vragen opperen. Het geeft ook een zicht op hoe het er in de toekomst zou kunnen (moeten) uitzien. De patiënt dient centraal te staan en behandeld te worden met respect voor zijn autonomie in een perfecte harmonie met de hem geboden zorg. De auteurs tonen ook aan dat een uniforme aanpak (evidence based therapy) en 'individueel gestuurde zorg' veelal hand in hand gaan. Ze wijzen ook op het 'never ending story' fenomeen. Zorg voor mensen blijft nu eenmaal een variabele in functie van de evolutie van de wetenschap en van de persoon in kwestie. Dit boek biedt verhelderende inzichten aan zowel al wie betrokken is bij de zorg voor mensen, de professionele zorgverstrekker, vrijwilliger, of beleidspersoon, als aan de patiënt zelf en zijn familie.

Abstract

In 1840, a French alienist, or psychiatrist, Joseph Guillaume Desmaisons Dupallans, visited a number of Italian asylums with the objective of discovering the best solution for the institute he was planning to set up in the vicinity of Bordeaux. This book comprises an Italian translation of the notes he made on this trip, conserved in a manuscript in the library of the former psychiatric hospital of San Salvi (Florence). Offering an extremely precise and detailed review of the structures visited, the work is a fascinating and important source for the reconstruction of the history of the institutes for the insane in Italy and also for the history of psychiatry itself. Nel 1840, un alienista francese, Joseph Guillaume Desmaisons Dupallans, compì un viaggio nei manicomi d'Italia per individuare quale potesse essere la soluzione migliore, a cui ispirare l'istituto che avrebbe costruito nei pressi di Bordeaux. Il volume pubblica in traduzione italiana gli appunti di tale ricognizione, conservati in un manoscritto presso la biblioteca dell'ex Ospedale psichiatrico di San Salvi (Firenze). Proponendo una rassegna estremamente precisa e dettagliata delle strutture visitate, l'opera rappresenta una fonte importantissima per la ricostruzione della storia degli Istituti per alienati in Italia e uno strumento per la stessa storia della psichiatria.

Abstract

In some parts of the world spending on pharmaceuticals is astronomical. In others people do not have access to basic or life-saving drugs. Individuals struggle to afford medications; whole populations are neglected, considered too poor to constitute profitable markets for the development and distribution of necessary drugs. The ethnographies brought together in this timely collection analyze both the dynamics of the burgeoning international pharmaceutical trade and the global inequalities that emerge from and are reinforced by market-driven medicine. They demonstrate that questions about who will be treated and who will not filter through every phase of pharmaceutical production, from preclinical research to human testing, marketing, distribution, prescription, and consumption. Whether considering how American drug companies seek to create a market for antidepressants in Japan, how Brazil has created a model HIV/AIDS prevention and treatment program, or how the urban poor in Delhi understand and access healthcare, these essays illuminate the roles of corporations, governments, NGOs, and individuals in relation to global pharmaceuticals.

Developments such as the increasing globalisation of the food industry, constant innovations in technologies and products, and changes in the susceptibility of populations to disease, have all highlighted the problem of emerging pathogens. Pathogens may be defined as emerging in a number of ways.
